Een **implementatiegids** (IG) legt vast hoe een land of sector de internationale standaard [HL7 FHIR](https://ehealth-atlas.be/fhir/) gebruikt: welke velden verplicht zijn, welke codes toegelaten zijn en hoe resources samenhangen. De Belgische IG's zijn de pakketten `hl7.fhir.be.*`: profielen, extensies, ValueSets en voorbeelden bovenop FHIR. Het [eHealth-platform](https://ehealth-atlas.be/ehealth-platform/) publiceert, wanneer nodig, specifieke FHIR-profielen en IG's voor federale initiatieven op ehealth.fgov.be/standards/fhir[^1]. Het principe is FHIR zo dicht mogelijk bij de basisstandaard te gebruiken en enkel Belgische nuances toe te voegen[^14]. Waarom ze nodig zijn: internationale FHIR-conformiteit alleen volstaat niet, systemen moeten dezelfde nationale afspraken gebruiken (terminologieserver, ValueSets, CareSets)[^15]. De IG's leggen die afspraken per domein vast, en elk [RIZIV](https://ehealth-atlas.be/riziv-inami/)-CareSet-initiatief moet vertrekken van FHIR-resources[^16]. Volgens de BIHR-principes van het RIZIV maakt FHIR-compliance alleen data niet interoperabel; conformiteit met de gedeelde profielen is vereist[^17]. De eerste fase (BE Core, Q4 2019) omvatte Patient, Practitioner, PractitionerRole, Organization, Observation, Provenance en AllergyIntolerance[^14]. ## Hoe is het opgebouwd? ### Beheer en publicatie HL7 Belgium-werkgroepen werken het datamodel en de profielen uit[^3]; een validatieteam van [HL7 Belgium](https://ehealth-atlas.be/hl7-belgium/) valideert de artefacten en komt momenteel tweewekelijks samen[^3]. Er zijn twee publicatieniveaus: HL7-communitystandaard via HL7 Belgium en federale standaard via het eHealth-platform[^3]. Het portaal gebruikt een kleurcode per type IG[^1]: 'Build' voor IG's in ontwikkeling[^1] en 'Federal' voor IG's die het eHealth-platform publiceert en als federale artefacten gevalideerd zijn[^1]. Er is ook een categorie 'IG's Published by HL7Be', maar geen van de 18 IG's valt daar feitelijk onder: ze hebben allemaal het eHealth-platform als uitgever[^1], en ook het internationale FHIR IG-register noemt het eHealth-platform als autoriteit[^18]. Een CareSet van [BeSafeShare](https://ehealth-atlas.be/besafeshare/) wordt als Belgisch FHIR-profiel gepubliceerd in de IG voor softwareleveranciers[^19]. Publicatie als federale standaard houdt niet automatisch een wettelijke verplichting in om ze meteen te implementeren[^3]. ### Versies, reeks en status Versie, releasedatum, FHIR-versie en publicatiereeks per IG komen uit het bestand `package-list.json` van elke IG. Daarin is 'Trial Use' de waarde van het veld `sequence` (de publicatiereeks), niet van het veld `status`: voor BE Core is `status` 'release', `sequence` 'Trial Use' en `fhirversion` 4.0.1[^4]. Alleen BE Lab heeft als `status` zelf 'trial-use'[^20]. Volgens de HL7-documentatie is `status` de levensfase van een versie en `sequence` een optionele groepering in een reeks[^21]; 'release' betekent een release zonder bijzondere status[^21], 'trial-use' een specificatie klaar voor gebruik op proef[^21], en sommige statuswaarden hebben enkel betekenis voor HL7 of andere standaardisatieorganisaties[^21]. Bij de Belgische IG's is 'Trial Use' dus de naam van de reeks; het statusveld 'release' zegt niets over formele rijpheid, behalve bij BE Lab. Een Belgische uitleg daarvan werd niet gevonden. De waarde `trial-use` in de frontmatter van deze pagina volgt de reeks. Alle 18 portaal-IG's hebben `fhirversion` 4.0.1; ook de CI-builds Glucose Monitoring en Patient Monitoring Outcome hebben fhirVersion 4.0.1[^22][^23]. Patient Monitoring Outcome blijft een CI-build van HL7 Belgium[^26]; een publicatieaanvraag werd niet gevonden. ### Terminologie SNOMED CT is de voorkeurscodering in de federale profielen[^27]; de ValueSets van BE Core Clinical worden via dte-tx.ehealth.fgov.be op basis van de SNOMED CT Belgian Edition geëxpandeerd[^28]. In BE Lab moeten laboresultaten in LOINC volgens de FOD-subset gecodeerd worden, anders ALBERT-codes[^29], met eenheden in UCUM[^29]. BE Core Clinical 1.3.0, BE Core 2.2.0 en BE Public Health 1.0.4 gebruikten dte-tx.ehealth.fgov.be/r4 als primaire terminologieserver bij publicatie[^30][^31][^32]; de oudere release BE Medication 1.1.1 gebruikte nog tx.fhir.org[^33], en in het buildrapport van BE PSS 1.1.0 gold de FOD-Ontoserver als gezaghebbende server voor SNOMED CT[^34]. Voor de CI-builds is de datum in het manifest een builddatum, geen releasedatum. ## Waar wordt het gebruikt? - **Vitalink en de kluizen**: de Vitalink-FHIR-documentatie voor softwareleveranciers bevat een sectie over de Be Public Health-IG voor de CVKO-bevolkingsonderzoeken naar kanker[^8], en secties over Be Vaccination en Be Allergy Intolerance[^8]. De drie regionale kluizen publiceerden een gemeenschappelijk FHIR-cookbook, eerst voor allergieën met het BeAllergyIntolerance-profiel[^70]. Immunisaties zijn het belangrijkste datatype in Vitalink FHIR[^71]. Welke IG-versie Vitalink ondersteunt, volgt niet uit de geciteerde tabeltekst, die enkel naar 'de laatst ondersteunde versie' verwijst[^8]. Zie [Vitalink](https://ehealth-atlas.be/vitalink/). - **Medicatie**: BE Medication definieert onder meer BeMedicationLine (de FHIR-tegenhanger van een medicatieschemalijn), BeMedicationDispense en BeMedicationPrescription[^45], en is ontwikkeld door de HL7 Belgium-community[^24]. Zie [Gedeeld medicatieschema](https://ehealth-atlas.be/medicatieschema/). - **MyCareNet ([eAgreement](https://ehealth-atlas.be/eagreement/))**: het eHealth-platform publiceert de IG Belgian MyCareNet Profiles samen met het MyCareNet-team als basis voor MyCareNet-use-cases[^9]. Het zijn vooral eAgreement-profielen: een generiek MessageHeader-profiel[^72] en Claim-profielen voor kinesitherapie en logopedie[^72]. Versie 1.0 (2021-07-01, FHIR R4) voegde de eerste eAgreement-profielen toe[^49]; 2.1.0 tot 2.1.2 verschenen op 2025-02-17 en 2.2.0 op 2026-04-24[^49], met in de releasenotes onder meer het schrappen van de waarde 'partialAgreement' en het codesysteem BeCSPartialAgreementValues[^73]. De CI-build toonde op 29 september 2026 nog 2.2.0[^74]. Voor technische richtlijnen verwijst de IG naar de niet-publieke SharePoint-omgeving[^75]. Zie [MyCareNet](https://ehealth-atlas.be/mycarenet/). - **Verwijsvoorschrift**: het RIZIV eReferral-IG voor de eReferral-API van [UHMEP](https://ehealth-atlas.be/uhmep/) noemt in zijn woordenlijst het federale DRP-IG als ouder-IG[^76], terwijl het portaal DRP als deprecated ten gunste van Referral markeert[^2]; technisch hangen de eReferral-CI-builds 2.0.0 en 2.1.0 af van Referral 1.0.0[^77]. Het Referral-IG 1.0.0 bevat enkel een algemeen, een verpleegkundig en een beeldvormingsprofiel, geen labo-specifiek voorschriftprofiel[^78]; een voorstel voor extra elementen voor het LAB2LAB-project staat sinds december 2025 als issue open[^79]. Het heeft een identifier-slice SSIN met kardinaliteit 0..* en de vaste system-URI https://www.ehealth.fgov.be/standards/fhir/core/NamingSystem/ssin; de invariant be-inv-SSIN eist dat een niet-gepseudonimiseerd SSIN uit 11 cijfers zonder leestekens bestaat[^10]. Het BeSSINNamingSystem noemt het identificatiesysteem 'NISS/INSZ', uitgegeven door het eHealth-platform, met OID 2.16.840.1.113883.3.6777.5.1[^80]. De guidance geeft een voorbeeld van een canonieke identifier en verwijst voor de precieze URI's naar de profielen[^81]; het voorbeeld is bedoeld voor een rijksregisternummer[^81]. Door de kardinaliteit 0..* verplicht BE Core het INSZ niet als patiëntidentificator, maar legt het enkel vast hoe het uitgedrukt wordt; specifieke IG's kunnen strenger zijn[^10]. ## Wat betekent het voor een ziekenhuis? - **Geen plicht, wel de referentie.** Publicatie als federale standaard verplicht niet tot implementatie[^3], maar volgens de BIHR-planning zouden EPD-systemen vanaf 2028 de informatie uit de CareSet-datamodellen moeten kunnen bevatten voor uitwisseling via FHIR[^15].
