De Patient Monitoring IG van [HL7 Belgium](https://ehealth-atlas.be/hl7-belgium/) geeft richtlijnen om patiëntmonitoringdata te integreren; belanghebbenden zijn EPD-systemen, transmurale dataproviders en thuisverpleging.[^4] De IG is (september 2026) geen geautoriseerde publicatie maar een continuous build van versie 0.1.0.[^2] In de configuratie staat de status op 'draft', met datum 2024-11-20, versie 0.1.0 en FHIR-versie 4.0.1.[^1] De IG werkt met twee bouwstenen: - **Carepath**: legt vast welke datapunten een transmurale dataprovider (bv. een thuismonitor) naar een zorgsysteem (EPD, command center, virtuele afdeling) moet sturen als onderdeel van het zorgpad.[^4] Daarbij horen ook de [SNOMED CT](https://ehealth-atlas.be/snomed-ct/)- en [LOINC](https://ehealth-atlas.be/loinc/)-codes; de inhoud is in ontwikkeling en nog niet finaal.[^3] - **Careset**: een gestructureerd verslag tussen EPD's of zorgactoren dat de semantiek van de carepaths hergebruikt en uitbreidt met onder meer ziekenhuisinstructies, contacten, vragenlijsten en uitkomsten, bijvoorbeeld bij [Thuishospitalisatie](https://ehealth-atlas.be/thuishospitalisatie/) tussen ziekenhuis en thuisverpleging.[^4] ## Welk probleem lost het op?
