HL7 FHIR (Fast Healthcare Interoperability Resources) is een standaard van de internationale HL7-organisatie voor het uitwisselen van zorginformatie, opgebouwd uit resources: kleine, identificeerbare blokjes informatie (zoals 'patiënt') die in verschillende contexten bruikbaar zijn[^1]. Deze pagina is bewust kort; de Belgische profielen staan op [Belgische FHIR-implementatiegidsen](https://ehealth-atlas.be/be-fhir-igs/). ## Welk probleem lost het op? In België moet FHIR de moderne opvolger worden van [KMEHR](https://ehealth-atlas.be/kmehr/): nieuwe gegevensstromen gebruiken bij voorkeur FHIR en nieuwe interfaces bij voorkeur FHIR REST[^2]. Het [RIZIV](https://ehealth-atlas.be/riziv-inami/) benadrukt dat voldoen aan internationale FHIR-normen alleen niet volstaat: systemen moeten ook dezelfde nationale afspraken gebruiken (terminologieserver, ValueSets, CareSets)[^3]. In de BIHR-architectuurprincipes (maart 2026) is die taakverdeling expliciet: FHIR regelt de syntax van de uitwisseling, CareSets, ValueSets en bindings regelen de semantiek[^4]. De Belgische profielen worden uitgewerkt in werkgroepen van HL7 Belgium[^11], de Belgische affiliate van HL7 International, erkend in april 2020[^12]. ## Belgische toepassing - **Beleid**: FHIR is de voorkeursstandaard sinds het actieplan e-Gezondheid 2019-2021[^2]. In 2019 besliste de programmaraad van het actieplan 2019-2021 dat elk RIZIV-careset-initiatief moet vertrekken van FHIR-resources[^13]. Binnen [BIHR](https://ehealth-atlas.be/bihr/) wordt FHIR de uitwisselingsstandaard conform Belgische profielen; volgens de planning zullen nieuwe en aangepaste uitwisselingen vanaf 2028 via FHIR verlopen[^3]. - **Profielen**: het eHealth-platform publiceert Belgische FHIR-profielen en IG's op ehealth.fgov.be/standards/fhir[^14], zo dicht mogelijk bij de basisstandaard[^15]. Zie [Belgische FHIR-implementatiegidsen](https://ehealth-atlas.be/be-fhir-igs/). - **Vitalink**: de FHIR-omgeving ging op 22/03/2024 in productie naast de KMEHR-omgeving, met REST in plaats van SOAP en I.AM Connect[^7]. Immunisaties zijn het belangrijkste datatype; ook de CVKO-kankerscreenings lopen via FHIR[^16]. [Vaccinnet 2.0](https://ehealth-atlas.be/vaccinnet/) gebruikt Vitalink en FHIR[^17]. - **Kluizen samen**: Vitalink, Abrumet en RSW publiceerden een gemeenschappelijk FHIR-cookbook, eerst voor allergieën met BeAllergyIntolerance[^18]. - **Medicatie**: in de RIZIV-planning (fase 2026-2027, acties voorzien in 2026) ondersteunt het RIZIV de kluizen en softwareleveranciers bij de implementatie van FHIR voor medicatiegegevens[^19]; dit is een planning (zie [Gedeeld medicatieschema](https://ehealth-atlas.be/medicatieschema/)). ### KMEHR→FHIR-transitie in het kort De overgang is geleidelijk: bestaande stromen worden aangemoedigd te migreren, 'best effort'[^13], en een 'big bang' is volgens het eHealth-platform niet mogelijk[^2]. KMEHR-stromen blijven binnen BIHR als alleen-lezen ondersteund[^3]. Volgens een RIZIV-presentatie (maart 2026, planning) wordt FHIR operationeel en KMEHR read-only bij een gecoördineerde omschakeling; op de slide staat 'Point T = 12 maanden na de publicatie van de laatste patient summary CareSet' als voetregel bij die stap[^20]. Of Point T voor alle stromen geldt of enkel voor de patient summary, is niet gespecificeerd. Het digitaal verwijsvoorschrift (eReferral) heeft een gepubliceerde FHIR-CareSet, Digital Referral Prescription (DRP); **interpretatie**: omdat de bron geen KMEHR-variant vermeldt, lijkt deze stroom van papier rechtstreeks naar FHIR te gaan, zonder KMEHR-tussenstap[^21]. Details en open punten staan op [KMEHR](https://ehealth-atlas.be/kmehr/); de planning per gegevensstroom staat op [BIHR (Belgian Integrated Health Record)](https://ehealth-atlas.be/bihr/). ## Praktische betekenis voor ziekenhuizen - **Vaccinnet 2.0 en Vitalink (canonieke passage)**: volgens de Vlaamse minister (antwoord op schriftelijke vraag nr. 327 van 5 februari 2025) zullen ziekenhuizen met Vaccinnet 2.0 (Vitalink FHIR) voor het eerst ook gegevens schrijven in Vitalink[^22]; op dat moment konden ze Vitalink enkel raadplegen[^22]. De productiestatus voor ziekenhuizen is niet apart geverifieerd. Op de derde BE FHIR-A-THON (september 2025) werkten 14 integratoren, waaronder ziekenhuizen, aan Immunisation in Vitalink@FHIR[^23]. - Volgens de huidige BIHR-planning zouden EPD-systemen vanaf 2028 de CareSet-datamodellen moeten kunnen bevatten zodat die via FHIR uitwisselbaar zijn[^3]; dat is een planning, geen vastgestelde verplichting. - De BMUC-voorwaarden spreken van delen in herbruikbare vorm (CareSets, FHIR, SNOMED CT)[^24] (zie [BMUC: financiering van het geïntegreerd EPD](https://ehealth-atlas.be/bmuc-epd-criteria/)). - **Aanbeveling**: beoordeel FHIR-koppelingen van leveranciers op conformiteit met de Belgische profielen en CareSets, niet alleen op 'FHIR-compatibel' (zie de RIZIV-principes hierboven). - **Aanbeveling**: kies R4 (4.0.1) met de Belgische profielen als doelversie voor integratie. Er is geen Belgische IG op R5/R6, maar kennis van R5 blijft nuttig omdat o.a. BE Medication R5-elementen via cross-version-extensies in R4 gebruikt. ## Implementatievoorwaarden en beperkingen FHIR-toegang tot Vitalink vraagt REST, I.AM Connect[^7] en pseudonimisering via het eHealth-platform[^7]. Publicatie als federale standaard houdt niet automatisch een wettelijke verplichting in om die meteen te implementeren[^11]. Een omzetting zonder betere codering levert weinig op[^2]; voor gecodeerde profielen is [SNOMED CT](https://ehealth-atlas.be/snomed-ct/) de voorkeurscodering[^25]. ## Onzekerheden en tegenstrijdige informatie - De publicatiedatum en de normatieve/STU-mix van R4 staan in het onderzoek, maar het citaat kon niet geverifieerd worden; ze worden hier niet als feit vermeld. - Er werd geen Belgische IG op R5 of R6 gevonden; dat betekent niet dat er geen plannen bestaan. - Of de ziekenhuishubs al FHIR aanbieden is niet onderzocht. - **Voorkeur, geen verplichting gevonden**: het eHealth-platform formuleert FHIR als voorkeursstandaard ('preferably') voor nieuwe gegevensstromen[^26]; een leveranciersstelling dat de KMEHR→FHIR-migratie al 'reglementair vastgelegd' is, wordt door geen gevonden wettekst gedekt (stand 2026-10-01)[^27].
