ICPC (International Classification of Primary Care) werd ontwikkeld door de WONCA International Classification Committee (WICC) en in 1987 voor het eerst gepubliceerd door Oxford University Press[1]. Het is een classificatie, geen referentieterminologie: ICPC-2 classificeert de contactreden, de behandelde problemen/diagnosen en interventies, en ordent die in een episode-of-care-structuur[2]. Volgens de SNOClass-brochure werd ICPC-3 in drie en een half jaar ontwikkeld, met betere koppelingen naar ICD-11, ICF, ICHI en SNOMED CT[9]. De codeerregels van ICPC-3 bouwen voort op die van ICPC-2[11]. Domus Medica noemt ICPC-1 en ICPC-2 verouderd; dat was de aanleiding voor ICPC-3[8]. De Franse vertaling (CISP-3) werd gemaakt door leden van de Franstalige CISP Club[12].

Huisartsen. Volgens eHealth wordt ICPC-2 in België door huisartsen gebruikt voor codering, in combinatie met ICD-10 (zie ICD-10-BE)[2]. Dat gebeurt meestal via de huisartsenthesaurus: 3BT is volgens de FOD de meest gebruikte interfaceterminologie in de Belgische huisartsgeneeskunde, en meer dan 51.000 termen hebben een IBUI-code en een mapping naar ICD-10 en ICPC-2[13]. De thesaurus koppelt elk tweetalig concept aan minstens één paar ICPC-2/ICD-10-codes[14]. Procedure- en interventieconcepten zijn niet aan ICD gekoppeld, maar enkel aan één ICPC-code[14].

Registratiecriteria. De registratiecriteria 2019 voor huisartsensoftware vereisen dat de software in de eerste plaats naar referentietabellen verwijst, waaronder IBUI/Thesaurus en ICPC-2/ICD-10-CM[15]. De software moet minstens ICD-10-, ICPC-2- en ATC/CNK-codes kunnen lezen en integreren[15]. De Evidence Linker (koppeling van diagnosen aan CEBAM-richtlijnen) wordt in huisartsensoftware getriggerd door ICPC-codes[15].

Uitwisseling. In SUMEHR 2.0 worden voor risico's en antecedenten IBUI, ICPC-2 en ICD-10 als codificaties aanbevolen[16]. De KMEHR-tabel ICPC (versie 1.0, 05/11/2009) bevat slechts een klein illustratief uittreksel[2]. In de federale FHIR-IG Prescription Search Support (v1.1.0; zie Belgische FHIR-implementatiegidsen) bevat een ValueSet codes uit ICPC-2, naast ICD-10-CM en SNOMED CT[17].

Overgang naar SNOMED CT. Volgens Domus Medica koos de Belgische overheid voor SNOMED CT om het informatieverlies tussen ICPC en ICD te vermijden[4]. De uniforme 3BT is een eenmalige versie die niet verder evolueert; de huisartsenthesaurus werd als General Practitioner Reference Set in de Belgische SNOMED CT-extensie geïntegreerd[13]. Het Terminologiecentrum meldde in januari 2021 dat de GP RefSet beschikbaar was[18]. Bij de lopende samenwerkingsprojecten van het Terminologiecentrum (maart 2025) horen maps 3BT–SNOMED CT, GP subset–ICD-10 en GP subset–ICPC-2[19], en SNOMED CT–ICPC-3[19]; of die maps al gepubliceerd zijn (bv. via de Belgische terminologieserver (FHIR)), werd niet gevonden; een UGent-project (1 januari 2024 – 31 december 2025) ontwikkelt daarvoor een mappingproces[10]. SNOMED International meldt dat zijn eigen map van SNOMED CT naar ICPC gedeprecieerd is[20].

Beleid. De B-IHR-conceptnota beoogt codering volgens onder meer SNOMED CT, ICPC-3, ICD-11 en LOINC[21]. Voor de EHDS vraagt België per relevant veld één Europese waardenset, bij voorkeur SNOMED CT, met mapping naar onder meer ICPC-3[22].
