SNOMED CT is een klinische **terminologie**: een verzameling concepten (diagnoses, procedures, bevindingen, lichaamsdelen, organismen) met onderlinge relaties, zodat software de betekenis van een registratie kan verwerken. Dat verschilt van een statistische **classificatie** zoals [ICD-10-BE](https://ehealth-atlas.be/icd-10-be/), die gevallen in groepen indeelt voor rapportering. SNOMED CT wordt internationaal beheerd door SNOMED International; België is lid sinds 2013[^1]. Volgens de roadmap van het Terminologiecentrum (maart 2026) koos België SNOMED CT als referentieterminologie waaruit alle andere codesystemen, zoals classificaties, afgeleid kunnen worden[^2]. Volgens de guidance bij de federale FHIR-profielen (Federal Core Profiles v2.1.1) is SNOMED CT de voorkeurscodering in de Belgische context, ondersteund door de FOD Volksgezondheid[^5]; dat is een uitgangspunt in de guidance, geen normatieve binding per profiel. Namen van micro-organismen en lichaamsdelen in laborapporten zijn beter vertegenwoordigd in SNOMED CT dan in [LOINC](https://ehealth-atlas.be/loinc/)[^14]; beide terminologieën vullen elkaar aan. ## Hoe werkt het? ### Belgische editie De Belgische release heeft een eigen deelpagina, [SNOMED CT Belgische editie (releases, refsets, BE Maps)](https://ehealth-atlas.be/snomed-ct-belgische-editie/), met releasekalender, inhoud, refsets, BE Maps en BE Drug Extension. Kort: de eerste Belgische release dateert van 15 maart 2018[^1]; sinds november 2024 publiceert België een volledige editie in plaats van een extensie[^15]. Volgens de Release Policy (metadata juni 2025) verschijnt ze elke maand op de 15de, behalve in januari en augustus[^3]; oudere bronnen noemen nog halfjaarlijkse releases[^16]. De editie van september 2026 (20260915, status PRODUCTION) bevat Nederlandse, Franse en Duitse vertalingen[^4], 269 nieuwe internationale concepten en geen nieuwe Belgische[^4]. Ze telt 634.074 actieve Nederlandse beschrijvingen[^4]; sinds maart 2026 worden die anders geteld, zodat oudere cijfers niet vergelijkbaar zijn[^4]. **Refsets** zijn deelverzamelingen voor een bepaald gebruik: er zijn er voor klinische registratie in EPD-software (onder meer medische problemen, cardiologie, hematologie, infectieziekten en intensieve zorg) en voor data-interoperabiliteit[^4]. In de subset voor medische problemen vond de validatie 70 ongeldige records, met een correctie gepland vóór de release van november 2026[^4]. ### Beheer en licentie Het Terminologiecentrum van de FOD Volksgezondheid is het National Release Center (NRC) en het enige contactpunt voor licenties en ontwikkeling van SNOMED CT in België[^1]. Het multidisciplinaire team hoort bij de Standards Unit binnen de dienst Data en Beleidsinformatie van het DG Gezondheidszorg[^16]; het verdeelt en beheert SNOMED CT in België en maakt de Nederlandse en Franse vertalingen[^16]. Externe partijen die inhoud aanleveren, dienen die uiterlijk op de laatste werkdag van de maand vóór de volgende release in[^3]. Door het lidmaatschap kunnen ontwikkelaars, leveranciers, kopers en gebruikers van e-healthsoftware in België SNOMED CT gratis gebruiken, maar enkel als licentiehouder onder licentievoorwaarden[^1]. Licentiehouders downloaden de release via de MLDS-dienst van SNOMED International[^1]; wie de Belgische core-clinical FHIR-specificaties implementeert, moet over een Affiliate-licentie beschikken[^17]. ### Terminologieserver Een **terminologieserver** stelt de codes en hun relaties via een API ter beschikking aan software. De FOD documenteert een nationale server op basis van Ontoserver[^18]; het Terminologiecentrum verspreidt de editie sinds 2025 ook via die server, naast MLDS[^15]. Volgens de roadmap is de nationale server een verspreidingsbron; EPD-pakketten en grote ziekenhuizen hebben een eigen ingebouwde server nodig[^19]. Het NRC stelt met het eHealth-platform een docker image met een lokale server ter beschikking, maar leveranciers mogen een eigen server gebruiken (stand november 2025)[^20]; [snowstorm-local (lokale terminologieserver van het eHealth-platform)](https://ehealth-atlas.be/snowstorm-local/) laadt bij het opstarten de Belgische en internationale editie[^21]. Een officiële Belgische map naar [ICD-10-BE](https://ehealth-atlas.be/icd-10-be/) werd niet gevonden: de enige SNOMED CT-naar-ICD-map op de server dte-tx is een HL7-voorbeeld (ICD-10-CM, ulnafracturen)[^22], en de BE Map vanuit de huisartsen-refset gaat naar 'ICD-10' zonder precisering[^12]. De FOD noemde in 2014 mapping naar ICD-10-CM en ICD-10-PCS al als aandachtspunt[^23]. De GP-refset voorziet mappings naar ICPC-2 en ICD-10, op termijn ICPC-3 en [ICD-11](https://ehealth-atlas.be/icd-11/)[^26]; ICD-11 zou via een mapping vanuit SNOMED CT geïmplementeerd worden, zonder datum[^2]. ## Waar staat het in België? - **FHIR-profielen**: de Body Site-ValueSet in de HL7 Belgium core-clinical IG (v1.3.0, STU1) wordt geëxpandeerd via de terminologieserver dte-tx.ehealth.fgov.be op basis van de Belgische editie[^27]. Zie [Belgische FHIR-implementatiegidsen](https://ehealth-atlas.be/be-fhir-igs/) en [HL7 FHIR](https://ehealth-atlas.be/fhir/). - **CareSets en kluizen**: BeSafeShare ([RIZIV](https://ehealth-atlas.be/riziv-inami/)) ontwikkelt gestructureerde, gecodeerde informatiesets[^6], met ValueSets in SNOMED CT overal waar dat waarde toevoegt[^6]; de CareSets worden via de regionale kluizen uitgewisseld, voor Vlaanderen [Vitalink](https://ehealth-atlas.be/vitalink/)[^6]. Voor vaccinaties is dat productie: de Vitalink FHIR-omgeving ging op 22/03/2024 in productie[^7] en immunisaties zijn er het belangrijkste datatype[^28]; de patient summary is planning (zie [BIHR: CareSets en EHDS-roadmap](https://ehealth-atlas.be/bihr-caresets/)). - **BIHR**: de B-IHR-conceptnota (2023) beoogt codering volgens SNOMED CT, ICPC-3, ICD-11, [ATC](https://ehealth-atlas.be/atc/), LOINC en ICF[^29]; in de RIZIV-planning bewaakt vooral SNOMED CT de semantische samenhang tijdens de overgang van KMEHR naar FHIR[^30], en volstaat FHIR alleen niet zonder dezelfde nationale afspraken[^31]. Zie [BIHR (Belgian Integrated Health Record)](https://ehealth-atlas.be/bihr/). - **Vaccinatie**: de Vaccinnet 2.0 Business Rules (v1.2.2, BR-13) vereisen voor elke immunisatieregistratie een geldige SNOMED-code als VaccineCode[^8]. Zie [Vaccinnet](https://ehealth-atlas.be/vaccinnet/). - **Huisartsensoftware**: volgens Le Spécialiste (4 juni 2026) moet tegen eind 2026 een SNOMED CT-terminologieserver in alle huisartsensoftware geïntegreerd zijn[^32]. Zie [Registratie (homologatie) van huisartsensoftware](https://ehealth-atlas.be/registratie-huisartsensoftware/). - **Ziekenhuisfinanciering**: BMUC-hubvoorwaarde d) (vanaf 1/7/2025) vraagt delen via hubs-metahub in herbruikbare vorm (CareSets, FHIR, SNOMED CT)[^10]. Zie [BMUC: financiering van het geïntegreerd EPD](https://ehealth-atlas.be/bmuc-epd-criteria/). - **Secundair gebruik en Europa**: de richtlijn om SNOMED CT te gebruiken stond al in het eHealth-actieplan 2013-2018, en healthdata.be neemt SNOMED CT maximaal over[^33]; in de [EHDS](https://ehealth-atlas.be/ehds/)-context pleit België voor één Europese waardenset per veld, bij voorkeur op basis van onder meer SNOMED CT en LOINC[^34]. ## Planning Het Terminologiecentrum stelde op de BIHR/eHealth Interoperability Day van 24 maart 2026 een 'SNOMED CT-implementation Roadmap' voor, als onderdeel van de EHDS-roadmap; het is een presentatie, geen regelgeving[^2]. Einddoel is gestructureerde en gecodeerde uitwisseling van Patient Summary, e-voorschrift en e-aflevering tegen maart 2029[^2]. Op 27 april 2026 was een kick-off met de EPD-leveranciers gepland, met de integratie van de terminologieserver en de terminology binding op de agenda; de roadmap beperkt zich niet tot huisartsensoftware[^2]. Ook de registratie van aangepaste EPD-software stond op de agenda, zonder te zeggen welk kader voor welk type EPD geldt[^2]. Volgens het Terminologiecentrum (juli 2026) ondersteunt het programma zorgverleners in de eerste én tweede lijn[^9]. ## Wat betekent het voor een ziekenhuis? - **Verplichting of planning?** Een wet, KB of MB die ziekenhuizen verplicht SNOMED CT te gebruiken, werd niet gevonden; het overzicht per verplichtingsgraad staat op [Verplichting of planning? Rechtsgrond voor SNOMED CT, CareSets en FHIR](https://ehealth-atlas.be/rechtsgrond-fhir-snomed/). De enige vermelding in een KB is de financieringsvoorwaarde BMUC d), zonder datum[^11]; de mijlpalen tot Q1 2029 zijn planning van de FOD[^36]; het artsenakkoord 2026-2027 noemt een koppeling voor hoofdstuk IV enkel 'op termijn'[^37]. De casestudy van SNOMED International die een plicht 'vanaf januari 2029' noemt, steunt op beleidsdocumenten[^38][^39]; de datum valt samen met de EHDS-termijn van maart 2029[^9]. - **Beleidsplanning die ook de tweede lijn noemt**: lokale terminologieserver rond eind 2026, gecodeerde probleemlijst rond 2027 Q3, gecodeerde Patient Summary tegen maart 2029[^2][^9]. De BMUC-probleemlijst zelf vereist geen SNOMED CT-codering[^11]; een pilot voor een SNOMED CT-probleemlijst via FHIR is [BE Patient Problemlist (SNOMED CT-probleemlijst via FHIR)](https://ehealth-atlas.be/be-patient-problemlist/)[^40]. - **Licentie**: wie de Belgische core-clinical FHIR-specificaties implementeert, heeft een Affiliate-licentie nodig[^17]. - **Specifieke stromen**: Vaccinnet 2.0 vereist SNOMED-gecodeerde vaccins[^8]; MZG-codering blijft ICD-10-BE zolang een officiële map ontbreekt[^12]. - **Aanbeveling** (eigen): vraag de EPD-leverancier naar zijn plan voor de roadmapstappen (lokale terminologieserver, gecodeerde probleemlijst), welke Belgische editie en refsets de software laadt en hoe vaak die wordt bijgewerkt; leg vast wie de licentie beheert; gebruik een map naar ICD-10-CM of WHO-ICD-10 hooguit als codeerhulp met menselijke validatie, nooit als automatische MZG-codering. ## Onzekerheden en tegenstrijdige informatie - **Verplicht vanaf januari 2029?** De snomed.org-casestudy's stellen dat Belgische ziekenhuizen vanaf januari 2029 diagnoses, voorgeschiedenis en procedures in SNOMED CT moeten registreren[^39][^38], met beleidsdocumenten als grondslag[^38]; deel 1 spreekt zelf van 'national policy'[^39]. De tijdlijn is een figuur die visueel gelezen werd. - **Geldt de roadmap voor ziekenhuizen?** Als beleidsplanning wel, want ze spreekt algemeen over EPD's en noemt de tweede lijn[^2][^9]; een registratiekader per type EPD, de gepubliceerde EPD-requirements en een verslag van de kick-off werden niet gevonden. - **Map naar ICD-10-BE**: niet gevonden; of de internationale WHO-ICD-10-map in de Belgische editie zit, is niet bevestigd[^4]. - **Versie op de servers**: de editie 20260915 heeft status PRODUCTION[^4], maar de publieke metadata van de FOD-Ontoserver toonden eind september 2026 nog 20251215 als recentste Belgische versie (eigen vaststelling).
